Evidence is part of the output, not an afterthought.

Design traceability into the workflow from the first handoff.

A summary is more useful when a reviewer can find the documents, records or calculations behind it. Evidence should travel with the work, rather than being assembled after a question or incident.

For document intelligence, retain source references and the relevant extracted fields. For reporting, preserve calculation definitions and input lineage. For research, distinguish quoted facts, interpretations and unresolved questions.

Log material handoffs with the role, action, time and decision. Avoid collecting unnecessary personal data or sensitive content in logs. Access and retention should reflect the purpose of the record.

A human approval gate works best when the reviewer can quickly see the proposed action, supporting checks, exceptions and remaining uncertainty. Too much unstructured information can obscure the decision as much as too little.

Build the evidence model alongside the workflow. The result is easier to challenge, maintain and improve.
